Health Records Release of Information

You may request access to your personal health information within a reasonable time, upon presentation of a written request and satisfactory identification. Details of this process are outlined below. 

We may charge you a fee for this service and if so, we will give you notice in advance of processing your request. See the fee schedule below for more information.

You have the right to access your personal health information. We may deny your request for access to your personal health information if a legal exception applies under the Personal Health Information Protection Act, 2004. If your request is denied, we will advise you in writing of the reason for the refusal and you may then challenge our decision. 

Please provide trueNorth Medical Centres 30 days to respond to your request. Please note that our services are centralized at the Dalhousie location and are responsible for releasing information for all clinics throughout Ontario.

+ How to request access to personal health information if I am NOT the patient (e.g. circle of care, health insurance company, lawyer, etc.)

Personal health information can only be released to a third party if signed consent from the patient is provided. In some instances, disclosure without consent may be authorized by the Personal Health Information Protection Act, 2004, Mental Health Act, or other applicable legislation.

trueNorth Medical CentresAuthorization for Release of Information form or equivalent consent must be provided (unless exemption applies).

You may choose one of the following methods of your preference:

  1. Complete your request through 🔗 medchart, a secure online portal or;
  2. Fax completed and signed consent to 416-981-8939 or;
  3. Mail completed and signed consent to 24 Dalhousie Street, Unit 201, Toronto ON M5B 2A5.

+ How to access records of a deceased patient

Provide proof of legal authority with a written request outlining the records required. You must provide a copy of the deceased person’s will naming you as the executor, or Certificate of Appointment of Estate Trustee signed by a registrar or the court. The latter may be obtained from the Estate Registrar Office at 393 University Avenue.
